We strive to keep our office as paperless and efficient as possible.
After you schedule your appointment, you’ll receive a text from our system with a link to complete your forms and upload your photo ID and insurance card electronically. You can even take care of your copay ahead of time. Completing everything before you arrive helps ensure a smooth, stress-free check-in, allows us to bring you back right away, and gives us more time to focus on you and your care.

What if I didn’t receive a text or can’t complete my paperwork ahead of time?

No problem at all. We simply ask that you arrive about 15 minutes early. When you check in, we’ll hand you an iPad to complete your forms, or a classic paper clipboard if you prefer. We’ll make sure you’re taken care of either way.
